Te Hauora Maioha

Te Hauora Maiona is contracted to provide:
Karakia
Mirimiri
Pastoral Care – Provide support and advocacy services to whanau who are accessing our services

Criteria - Who can access our services:
Whanau who have a current Community Services Card and will be of:
Maori (or)
Pacific (or)
Any other ethnicity living in NZ Deprivation 9-10

Whanau who meet the above criteria must also have a health problem that can be identified by:
· Your family doctor or nurse
· A Hauora Maori Provider
· Physiotherapist
· Specialist

To ensure whanau needs are fully met we do encourage you to:
· Visit your family doctor, nurse or health professional to discuss your health and wellbeing needs
· Discuss with your family doctor or nurse if Te Hauora Maioha services would complement current treatment
· Request a referral to the service – Once referral is printed you can present to the Maioha Team to book an initial appointment
· If at any stage the team at Maioha recommend you visit your family doctor/specialist you will need to make an appointment to see your doctor as soon as possible
